Cover Florida plans by United Health Care

In order to apply for benefits under Cover Florida Plans by United Health Care you must fill out the application and enrollent form for either individual or group. Then mail the originals to Powers Insurance, 22160 Tuckahoe Road, Alva, FL 33920. We must fill out a checklist and include with your application. Please contact us with any questions.
